Regarding Claims and Billing:

The Surgery Center of Farmington will send a separate claim to your insurance company for surgery, and for anesthesia charges. Once your insurance has paid its share you will receive a statement. You will receive separate statements for surgery and anesthesia charges from the Surgery Center. You may also receive statements from your physician and/or outside laboratory or pathology services.
